Summary Description: The Skilled Nursing Facility Certified Nursing Assistant (CNA) provides outstanding patient care and physical support for residents Activities of Daily Living (ADL). The CNA shall utilize knowledge of residents’ rights, legal/ethical concepts, communication skills, safety and infection prevention while performing the following: basic nursing skills, basic restorative skills, mental health and psychosocial skills, and nursing team member skills.
Supervision Received: Reports to the Director of Nursing (SNF).
Responsibilities: 1. Participates in patient and resident care through procedures within the scope of their training and practice, according to state and federal regulations and facility policies and procedures. 2. Assists in the preparation for admission for residents and prepares rooms for new admissions. 3. Assists in and accompanies residents in admissions, transfers, and discharge procedures. 4. Provides age and developmentally appropriate morning care, which may include bed bath, shower or whirlpool, oral hygiene, combing hair, back care, dressing residents, changing bed linen, cleaning overbed table and bedside stand, straightening room and other general care as necessary throughout the day. 5. Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri-care, freshening linen, and straightening rooms to ensure obstructions to movement is minimized. Follows residents Plan of Care. 6. Notifies appropriate licensed staff when resident complains of pain. 7. Provides post-mortem care. 8. Assists RN/LPN/LVN in treatments and procedures. 9. Using proper body mechanics and available resources; assists with care such as positioning, lifting, turning, ambulation, and applying/utilizing proper specialized equipment and tools to facilitate care. Assists in use of bedpan or commode and constantly checks for reddened areas or skin breakdown and reports to staff nurse. 10. Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors. 11. Supports and maintains a culture of safety and quality. 12. Assists with recording vital signs, height and weight, Input and Output, and other data as assigned, and reports variances to staff nurses. 13. Prepares resident for meals, distributes food trays, aids or feeds residents as needed being aware of dietary needs/restrictions of resident. Offers appropriate snacks and adequate hydration. 14. Ensures residents are dressed properly and used clothing is properly stored in the dirty clothes hamper/bag. Ensures that residents are always clean and dry. 15. Records observation of patient and resident behavior, including complaints and/or physical symptoms, reports issues and concerns to staff nurse/CNA Manager including change in residents’ status, response to care, unusual occurrences/observations, problems with completion of assignments, or any safety issues. 16. Participates in the development, implementation, and evaluation of residents’ care plans and protects residents from neglect and abuse. 17. Understands the policies on restraints their proper use. 18. Accompany residents to appointments, as directed. 19. Provides all the above general activities to residents in isolation. 20. Actively receives change of shift report and knows location of assigned residents. 21. Makes rounds with oncoming shift to ensure the unit is left in good condition and oncoming shift knows assigned residents and their status. 22. Promotes residents rights, provides interventions to increase residents’ quality of life and self-worth and involves residents in decisions affecting their care. 23. Encourages selfcare/independence with ADL’s implementing CNA interventions as noted on each person’s Care Plan. 24. Aware of rehabilitation goals of each resident and assists with implementation and documentation as required. 25. Attend staff development activities to improve knowledge and skills and to maintain competency and certification. 26. Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families. Functions as liaison between administration, residents, physicians, and other healthcare providers. 27. Adheres to established departmental policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention, and control standards. 28. Demonstrates the ability to be flexible, organized and function under stressful situations. 29. Maintains personal health and wellness to prevent transmission of illness to patients or residents. 30. Communicates the mission, ethics, and goals of the hospital. Complies with all organizational policies regarding ethical business practices. Represents the organization in a positive and professional manner. 31. Demonstrates responsibility for knowing and adhering to all District and departmental policies and procedures. 32. Understands the importance of resident confidentiality and HIPAA rules and regulations. 33. Other Duties as assigned.
Qualifications: 1. Current certification as a nursing assistant in the State of Wyoming. 2. Minimum of one-year certified nursing assistant experience preferred. 3. Current certification in Basic Life Support or certification within 3 months of hire.
Knowledge, Skills, and Abilities: 1. Possesses a genuine interest and concern for geriatric and disabled persons. 2. Knowledge of long-term care procedures and goals. 3. Knowledge of restraint policies and how that applies to individual care plans. 4. Ability to work as a member of the "health care team." 5. Knowledge of good body mechanics and proper lifting techniques. 6. Knowledge of performing and following infection prevention and control procedures. 7. Ability to follow instructions and ask questions as appropriate. 8. Ability to remain calm in stressful situations. 9. Ability to communicate clearly and effectively with residents, visitors, and coworkers.
Working Conditions: 1. Subject to varying situations including ability to understand and respond appropriately, effectively, and sensitively to special population groups, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc. 2. Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities. 3. Medical office, exam room, laboratory, radiology, emergency room and professional practice settings. Frequent exposure to communicable diseases and other conditions common to rural clinic environments. 4. The work environment involves exposure to unusual elements, potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. 5. May require travel between District sites.
Physical Demands: Varied activities include standing, walking, reaching, bending, and lifting. Requires full range of body motion including handling and lifting patients. Also requires manual and finger dexterity, and eye-hand coordination. It requires standing and walking for extensive periods of time. Occasionally lifts and carry items weighing up to fifty pounds. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.
